South Shore Hospital Honored with Innovator Award for Groundbreaking Work in Clinical Pharmacy Practice

South Shore Hospital, part of South Shore Health, has received an Innovator Award from the editors of Healthcare Innovation for its groundbreaking work in Clinical Pharmacy Practice by improving the medication reconciliation process.
The hospital received third-place for its ingenuity in developing a medication system aimed at reducing adverse drug events in medically complex patients.

Members of the pharmacy team at South Shore Hospital took ownership of tracking medication history in medically complex patients and transformed the organization’s reconciliation process, all on a tight budget. The result led to a significant reduction of adverse drug events in patients with serious and chronic illnesses.
