Home-Based Outpatient Rehabilitation Program Wins Home Care Alliance Innovation Award
South Shore VNA’s Home-Based Outpatient Rehabilitation Program has been recognized with an Innovation Award from the Home Care Alliance of Massachusetts, honoring its pioneering approach to advancing home health care.
The recognition was part of HCA’s 40th annual Our “Time to Shine” Awards, which celebrate home care programs and clinicians from across Massachusetts for their commitment to quality patient care and colleague well-being.
The Innovation Showcase honors groundbreaking programs that have had a positive impact on patients and the delivery of home care services in Massachusetts.
“We’re thrilled to see the Home-Based Outpatient Rehabilitation Team receive this deserved recognition for all of their hard work in bringing this program to life,” said Richard Pascarelli, Director of Revenue Cycle & Business Operations for South Shore Health’s Home Care Division. “Without our team’s vision, dedication, and forward thinking, this program would not have been possible.”
Launched in June 2024, the Home-Based Outpatient Rehabilitation Program offers physical therapy, occupational therapy, and speech therapy services to patients in the comfort of their homes, without requiring a homebound diagnosis.
The program is ideal for patients who would normally seek treatment at outpatient clinics but face barriers to care, including transportation issues, mobility challenges, or caregiver responsibilities.
Through the program, patients receive outpatient-level care from VNA therapists at home, with treatments tailored to their environment and unique circumstances.
The Home-Based Outpatient Rehabilitation Program features two dozen therapists who treat patients across the South Shore.
Since launch, the Home-Based Outpatient Rehabilitation Team has conducted more than 3,000 patient visits with an average of 50 referrals per month, both from within South Shore Health and from other practices in the community.
