South Shore Hospital’s Phase 2 Attestation Form

South Shore Hospital is committed to the safety of our community.
In order to comply with the Commonwealth of Massachusetts’ Phase 2 reopening plan, hospital and health care providers must meet a set of standards and guidelines outlined by the Massachusetts Department of Public Health (DPH).
Information about the Commonwealth reopening plan can be found on the Mass.gov website.
On June 8, 2020, South Shore Hospital submitted the Acute Care Hospital Phase 2 Reopen Attestation Form to the Massachusetts Department of Public Health (DPH).
By doing so, South Shore Hospital has attested compliance in accordance with DPH Hospital Reopening Guidelines. The completed form can be seen below.
