South Shore Health’s Home Care Division offers a variety of services for patients of every age. If you need help caring for a newborn baby, have had surgery, have an acute or chronic medical condition, are disabled, are living alone, or need help to stay independent and in your home, our home care team can help. 

From nursing and rehabilitation therapy to highly specialized care, to help with daily tasks, our South Shore VNA team is ready to care for you in the comfort of your home, twenty-four hours a day, seven days a week. 

Our Approach
Patient- and family-centered care guides all of our decisions as we care for the medical, social, emotional, and spiritual needs of our patients. We work closely with patients’ physicians and other community health providers to prevent and manage acute and chronic illnesses, so patients can avoid hospitalization and remain in their homes.
