South Shore Health Hospital at Home Celebrates 100th Patient Milestone

South Shore Hospital, part of South Shore Health, is proud to mark a significant milestone in its innovative Hospital at Home program, having successfully treated its 100th patient. 

This achievement underscores the program’s commitment to providing high-quality, patient-centered care in the comfort of patients’ homes.

“Launched just three months ago in June 2024, the South Shore Health Hospital at Home program is delivering high quality, inpatient-level care for certain patients in their homes, while enhancing patient experience, family involvement, and ultimately achieving improved patient outcomes,” said Timothy Lynch, Vice President, Clinical Services. 

The program, which services qualified patients from Quincy to Plymouth, has been particularly beneficial for individuals with chronic conditions, post-surgical needs, and those requiring post-acute care management.

“There’s nothing like being able to sleep in your own bed and be cared for that way,” said Sarah Paez, GNP-BC, Nurse Manager for Hospital at Home. 

"The program has been very well received by patients. Our team believes in what we are doing and patients can tell."

Key Highlights of the Program:

• Comprehensive Care: Patients receive a full spectrum of medical services, from a team of skilled nurses, paramedics and physicians, including daily visits, remote monitoring, and access to advanced medical equipment.  
• Patient-Centered Approach: The program focuses on personalized care plans tailored to each patient’s unique needs, ensuring they receive the best possible care in a familiar environment.
• Positive Outcomes: Since its inception, the program has demonstrated improved patient outcomes, reduced readmission rates, and high patient satisfaction scores.

One recent patient shared her thoughts when asked about the care she received from the South Shore Health Hospital at Home team: “Perfect. It was just like I rang for the nurse and she came. The team answered all my questions. They were very professional, informative and supportive.”

As South Shore Hospital celebrates this milestone, it remains committed to growing and expanding the Hospital at Home program to serve even more patients in the community.
