What is South Shore Health Hospital at Home?
South Shore Health Hospital at Home provides patients with certain medical conditions the opportunity to receive hospital-level care in the comfort of their own home. Patients are referred by a multidisciplinary team including a hospital-based provider.
What types of services will I receive through South Shore Health Hospital at Home?
When admitted to Hospital at Home, patients receive high quality, safe, and personalized care at home. A team of skilled nurses, paramedics and physicians will plan and oversee your care, administer medications, and treat your symptoms in the same way we do in the hospital. Equipment is provided to keep you connected to your care team. Members of the team will stay in touch with you and oversee your care needs and overall progress towards recovery.
You will receive all the medical care that you would otherwise experience as a patient on a unit at our hospital, including:
- Assessments and treatments in your home at least twice daily by our paramedics and nurses. They will bring all necessary supplies to care for you.
- Daily clinical assessments and review of your treatment plan with your hospital provider.
- 24/7 virtual care by our nursing team with frequent visits throughout the day.
- If needed, oxygen, blood draws, labs, and imaging to support your plan.
Additional Hospital at Home services may include physical and occupational therapy, social work, and spiritual care to best support your recovery needs.
How much does South Shore Health Hospital at Home cost?
Services provided on the South Shore Health Hospital at Home unit are billed through your insurance and, in many cases, will be covered similarly to a regular inpatient hospital stay. Our care managers can assist you with contacting your insurance company to determine any out-of-pocket costs.
Who qualifies for South Shore Health Hospital at Home?
Our program is ideal for patients who need hospital-level care and who are well enough to be treated at home. Members of your care team will provide an initial in-person medical review at South Shore Hospital to determine eligibility based on condition, home setting, and town/city in which patients live. Common conditions include:
- COPD
- Cellulitis
- Heart Failure
- Pyelonephritis (Kidney Infection)
- Diverticulitis
- Pneumonia
- Other appropriate conditions
What is a typical day in South Shore Health Hospital at Home?
Just as if you were staying on a patient care floor at the hospital, your care provider will see you throughout your home stay. Once at home, a South Shore Hospital at Home nurse or paramedic will see you in person a minimum of twice daily. Each patient will have a specific care plan and be monitored by the Hospital at Home care team. We will align this schedule with your own as best as possible.
Morning
Virtual visit with you and a Hospital at Home registered nurse (RN), including checking your vital signs and taking your morning medications through your portable computer.
Later morning
In-home visit by a Hospital at Home paramedic. Virtual visit with provider and care team through your portable computer.
Afternoon
In-home visit by a Hospital at Home paramedic to assist with medication administration and to recheck vital signs using your in-home equipment.
Evening
Virtual visit with a nurse, including checking vital signs, medication administration and education around your plan of care.
Nighttime
Nightly virtual visit and review of the next day’s schedule with a nurse prior to bedtime.
