South Shore Health System’s Foundation team has developed several tactics to help you refer patients to our Grateful Patient program. You choose the tactic that feels most natural to you.

How to Refer a Patient

1. Use Our Caregiver Referral Form

Our Caregiver Referral Form allows you to submit your patient’s information electronically. Once you submit the form, we’ll be in touch with any follow-up questions and will keep you updated throughout the process.

2. Contact Us and Make the Referral

The Foundation team can reach out to your patient directly once you make a referral. To refer a grateful patient or family member call (781) 624-8600 or email

3. Telephone or In-Person Discussion

The easiest way to make a referral is to talk to them directly. This sample script is a great outline of how the conversation could flow. Remember, it’s just a guide. You have a personal relationship with your patient or their family member and we want to be sure your natural personality comes through.

Contact Cards have been particularly helpful to caregivers. If you would like these business-sized cards sent to you directly, please contact the Foundation.

4. Write a Personal Letter, Note, or Email

A personal letter, hand-written note or an email from you to a potential grateful patient can be extremely effective. The documents below should be a great starting point. We recommend that you personalize them and add your own voice and personality. In addition, the Foundation team can also work with you to develop a plan for follow-up communications.
